Welcome to Sullivan STEAM Magnet School! You and your student(s) are invited to Sullivan on Thursday, May 9 from 8:00am to 10:30am to complete registration and meet the teachers! There will be staff available to help you with the forms and to answer questions. If you cannot come to Sullivan on this date, please come in sometime before June 13th during school registration hours of 9am-1pm. The registration process can be accessed at: https://enroll.exploremps.org/
‼ To complete the process, we will need the following information from you before the beginning of the 2024-25 school year:
1. Birth Verification (birth certificate, passport, baptismal record, or
immigration card)
2. Record of Immunizations (including immunizations required for entry to Kindergarten)
3. Recent physical exam, if available.
4. Emergency contact information (names, addresses, phone numbers)
5. Information on childcare arrangements (Address, phone number) for transportation purposes.
We will need ALL the items listed above before your child begins school. If your child attended preschool at Sullivan last year, we have this information on file, though you may need to bring an updated immunization record and a copy of their 5-year physical exam.
