Mat Pilates Class – Flow through an energizing Pilates session designed to sculpt and stretch your body, led by Pilattes founder Melanie McKenna. This class is for all levels, focusing on core strength, flexibility, and mindful movement. Beginners welcome.

Gua Sha Demonstration – Learn the art of Gua Sha and experience its soothing benefits as Heyday skincare experts guide you through a demonstration. You’ll leave with practical tips and techniques to incorporate into your skincare routine.

Social Hour – Stay afterwards to hangout at Senna House Pool Deck and get to know other wellness-loving attendees!

PILATTES WELLNESS LLC PARTICIPANT LIABILITY WAIVER AND RELEASE FORM

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.

By purchasing this ticket, I, the undersigned participant, acknowledge and agree to the following terms and conditions as a prerequisite to my participation in any event, activity, or gathering (“Event”) organized by PILATTES WELLNESS LLC (“Pilattes”).

ASSUMPTION OF RISK

I acknowledge that my participation in any Pilattes Event is voluntary and may involve inherent risks, including but not limited to physical activity, transportation before, during, and after the event, food and beverage consumption, interactions with other attendees, and exposure to weather conditions and unforeseen circumstances. I assume all risks associated with my participation, including injury, illness, property loss, or damage.

WAIVER AND RELEASE OF LIABILITY

I, on behalf of myself, my heirs, executors, administrators, and assigns, release, waive, discharge, and hold harmless Pilattes, its founders, officers, members, affiliates, sponsors, event partners, and any associated personnel from any and all claims, demands, damages, injuries, losses, or liabilities that may arise from my participation in any Event, whether caused by negligence or otherwise.

TRANSPORTATION LIABILITY WAIVER

I understand that PILATTES WELLNESS LLC is not responsible for any incidents, injuries, delays, or damages related to transportation before, during, or after an Event, including but not limited to driving a vehicle, rideshare services, carpooling, or public transit. Additionally, Pilattes is not held liable for any towing, parking expenses, or related costs incurred by participants.

MEDICAL TREATMENT

I consent to receive medical treatment in the event of injury, accident, or illness during a Pilattes Event. I understand that Pilattes does not provide medical insurance or coverage for participants and that I am responsible for my own medical expenses.

ACKNOWLEDGMENT OF HEALTH CONDITIONS

I confirm that I am in good health and physically able to participate in Pilattes Events. I acknowledge that it is my responsibility to consult a physician if I have any concerns about my health or ability to participate in any Event activities.

PERSONAL BELONGINGS DISCLAIMER

I understand that PILATTES WELLNESS LLC i s not responsible for any lost, stolen, or damaged personal belongings during an Event. I assume full responsibility for my personal items at all times.

PHOTOGRAPHY & MEDIA RELEASE

I grant PILATTES WELLNESS LLC permission to use any photographs, videos, or recordings taken of me during Events for promotional, marketing, or other business-related purposes without compensation or further approval.

CODE OF CONDUCT

I agree to conduct myself in a respectful and responsible manner while participating in Pilattes Events. I understand that any disruptive, inappropriate, or unsafe behavior may result in my removal from an Event without a refund and potential exclusion from future Events.

INDEMNIFICATION

I agree to indemnify and hold harmless PILATTES WELLNESS LLC from any claims, liabilities, damages, or expenses, including legal fees, arising from my actions or conduct at an Event, including damages to persons or property.

FORCE MAJEURE

I understand that PILATTES WELLNESS LLC is not responsible for any cancellations, modifications, or delays in an Event due to circumstances beyond its reasonable control, including but not limited to acts of God, natural disasters, pandemics, government restrictions, or other unforeseen events.

CANCELLATION POLICY

All ticket purchases for PILATTES WELLNESS LLC Events are final. We do not offer returns, refunds, or exchanges for any reason, including scheduling conflicts, weather conditions, or personal circumstances. If you are unable to attend a purchased Event, you may apply your credit toward another Pilattes Event within three months from the original purchase date, subject to availability. The new Event must not be fully booked. Credit is non-transferable and can only be used by the original ticket holder. Event credit does not guarantee entry and is based on availability. To apply your event credit, you must contact Pilattes at least 72 hours before the original Event start time. Requests submitted after the event has occurred will not be considered. Pilattes reserves the right to update or modify this Return Policy at any time. Changes will be posted on our website and will apply to future ticket purchases.

CONSENT FOR EVENT COMMUNICATION

By signing this waiver, I consent to receive communications from PILATTES WELLNESS LLC regarding Event details, updates, and future events via email or text.

PURCHASING TICKETS FOR OTHERS

If you are purchasing tickets on behalf of other individuals, you affirm that you have obtained their consent to agree to this Liability Waiver and Release Form. By completing the purchase, you accept responsibility for informing all ticket recipients of these terms and ensuring their compliance.

GOVERNING LAW

This waiver shall be governed and interpreted in accordance with the laws of the State of Arizona and any disputes arising from this agreement shall be resolved in the appropriate courts of Arizona.

SEVERABILITY

If any provision of this waiver is found to be unlawful or unenforceable, the remaining provisions shall remain in full force and effect.

ACKNOWLEDGEMENT AND ACCEPTANCE

I have read and fully understand the terms of this waiver and voluntarily agree to its provisions. By signing below, I confirm that I am at least 18 years of age and legally competent to sign this agreement.
