Frontlines of Care: 30 Years of Impact in Community Mental Health

For three decades, the UCSF’s Emergency Department Case Management (EDCM) program has been a lifeline for San Francisco’s most marginalized community members— people experiencing homelessness, severe mental illness, addiction, complex trauma, and serious medical conditions who frequently rely on the emergency department and/or medical admissions at San Francisco General Hospital. 

EDCM is the only fully integrated, interdisciplinary behavioral health case management program in San Francisco, uniting social work, psychiatry, and medicine to bridge gaps in care and build therapeutic relationships. Our work reduces emergency department reliance, strengthens community health connections, and provides dignity and stability where systems often fail.

At this special 30th anniversary gathering, we’ll reflect on: 

• What wraparound care really looks like for people who live at the intersection of homelessness, mental illness, and medical fragility. 

• Why this model works—and why it requires persistence, creativity, and collaboration. 

• Patient success stories that highlight resilience and recovery in the face of immense challenges.

The evening will begin with a conversation with program leaders and a Q&A (6–7pm), followed by time to mingle over food and drinks (7–8pm). 

This event is both a celebration and a call to action: an opportunity to honor three decades of community mental health, learn from the people who do this work every day, and invest in the care of San Francisco’s most vulnerable residents.
