FREE Sports Physical Event 1

Arizona student-athletes are required to complete a sports physical before participating in athletics. To help ensure access for all, Banner – University Medicine's Department of Orthopedics and Sports Medicine has arranged two **No-Cost Sports Physicals** events for our community's youth athletes.
**Required Forms**
A printed and completed AIA Examination Form and AIA Evaluation Form are required. All medical history questions must be completed by the athlete and parent/guardian before arrival. A parent/guardian signature is required on the last page of the AIA Evaluation Form.
A parent/guardian does not need to be present if all paperwork is completed and signed appropriately. Banner Sports Medicine staff will turn away athletes who do not have the required paperwork and signatures at check-in. Verbal permission will not be accepted.

**Important Information**
Physicals will be provided on a first-come, first-served basis. Athletes must be in line for registration by the posted closing time.
Patients and school personnel are responsible for all documentation provided during physicals. Banner Sports Medicine does not keep copies of these records.
