Get your student ready for the school year with a FREE Sports Physical at LaVie Health!
Whether they're playing football, volleyball, basketball, cheer, soccer, or participating in any school activity that requires a sports physical, we're here to help make it easy AND completely free.
Here's what you need to know:
🏈 FREE sports physicals for any school activity requiring an MHSA sports physical.
📅 Appointments are required (call 406-652-4868 to schedule)
👨‍👩‍👧 A parent or legal guardian must attend the appointment
📝 Please complete the MHSA Sports Physical Form before you arrive (instructions below)
🍧 Everyone will receive a FREE snow cone after their appointment from Kickin' Kones!
