🎀 FREE Mammogram Requirements
To receive a free mammogram, you must:
• Be 40 years or older
• Bring an ID or passport
• Have a Primary Care Provider (PCP) so results can be provided to them
• Be at least one year from your previous mammogram
• Provide information about your previous breast imaging
