What is Advance Care Planning (ACP)?
This workshop is for anyone wanting a "hands on" help with thier ACP - either to start or to finish.
Advance Care Planning is about thinking ahead, talking with your whānau and healthcare team, and making decisions about the care you want if you are unable to speak for yourself.
Learn more about what advanced care planning is, work through a tailored guide, and capture what matters most to you should you become unwell and unable to speak for yourself.
NOTE: This 1.5 hour workshop assumes you already know something about Advance Care Planning.
