Strengthening Connections Between Primary Care & Food Security Resources

We are excited to invite primary care providers to an upcoming Primary Care Network (PCN) event focused on strengthening connections between primary care and food security resources, helping you better support patients experiencing food insecurity.

Over the past year, the Vancouver Steering Committee has been leading a project to address gaps in integrating primary care providers with local food security supports. Through collaboration with community agencies and partners across Vancouver, we’ve been learning from their expertise and exploring practical ways to improve access, coordination, and referral pathways.

This May, join clinics, providers, and community partners for an evening of connection, shared learning, and relationship-building. Together, we will explore local resources, hear from community experts, and build confidence in discussing food security with patients and connecting them to meaningful support.

Our goal: to increase provider awareness, confidence, and comfort in identifying food insecurity and sharing relevant community resources.
