If you are under the age of 18, a parent or legal guardian must bring you to the clinic. If you are making a second dose appointment, your first dose of vaccine has to have been on or before September 9, 2021. This clinic is a drive thru clinic, you will remain in your car the entire time.
• To save time, please go to www.effcohealth.org to print and fill out the vaccine consent form prior to coming to the clinic to get your COVID vaccination.
• Please bring a photo ID with you.
• If you have insurance, please bring your insurance card. We may bill an administration fee to your insurance and that will be accepted as full payment, there will be NO CHARGE to the individual.
• Wear short sleeves
• Bring your own pen.
• Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
• If this is your second dose, please bring your vaccine record card you were given for your first dose.
You will be asked to stay for 15 minutes of observation after you receive the vaccine.
Final Directions
The clinic entrance is at the North East corner of the parking lot of the Effingham County Health Department.
There are no bathroom facilities available.
Please bring your pre-printed ticket, completed and signed consent form, and insurance card to your appointment.
If this is your second dose, please also bring your 1st dose vaccine record card.
PLEASE DO NOT COME EARLY FOR YOUR APPOINTMENT.
