Join us for a one-time, interactive presentation designed to support healthy aging and reduce the risk of falls, the leading cause of trauma for older adults in Sonoma County.This session begins with an introduction to Age-Friendly Healthcare, a national initiative from the Institute for Healthcare Improvement that’s changing how we care for older adults.
You'll learn the 4M Framework, a simple, powerful approach to improving health conversations with your doctor, and receive a take-home worksheet to guide those discussions.
From there, we’ll shift to practical strategies for fall prevention, including:
~ Identifying common fall risks at home and in the community
~ Behavior changes that reduce your risk of injury
~ Simple, effective exercises to build strength and balance
~ Emergency preparedness tips tailored for older adults

Presenter: Brooke Brand - Trauma Services Providence Santa Rosa Memorial Regional Trauma Center
Brooke has been a registered nurse since 1987, with over two decades of experience in Emergency Departments across the U.S. She came to Sonoma County in 2000 as a contract emergency and trauma RN, working at several local hospitals before joining Memorial’s Trauma Service as Program Manager for Injury Prevention in 2021. Now focused on outreach and education, she leads initiatives to reduce falls among older adults—the leading cause of trauma activations in the county.
