About this Event
Before creating an Advance Health Directive AHD, it is important to have a good understanding of what advance healthcare planning is and what is involved in the planning process. These are YOUR choices for YOUR health and personal care goals.
The creation of an AHD is the best way to make your preferences known regarding the type of treatment you would prefer, if you are unable to communicate or make decisions for yourself in a time that is often difficult for family members. An AHD is a way to respect your values, beliefs and preferences.
This informative session will focus on YOUR AHD, question time and the option of completing your personalised AHD, to be signed and authorised on the day.
Be brave. Be Curious. Be informed.
