GENERAL RELEASE FORM FOR ELECTRONIC REGISTRATION

By participating in Diamonds and Pearls Winter Gala you agree to adhere to all instructions, guidelines, and safety rules. In addition, you acknowledge that participation in the event exposes you to a possibility of personal injury. You, being fully aware that participation in the program expordses you to a possible risk of personal injury, hereby release Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., from any and all liability from property damage, personal injuries, or other claims arising from or in connection with your participation in the program including claims that are known and unknown, foreseen and unforeseen, future or contingent.

You further release Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., from any and all liability from any injury, damage or loss sustained by participants, guests or vehicles before, during or after the Diamonds & Pearls Winter Gala as a result of your participation in the event or associated events (this includes food or beverage consumption prepared and distributed).

By participating in Diamonds & Pearls Winter Gala, you authorize Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., and those acting for and with Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., to use interview, photograph, video and/or audio record you, without compensation in any medium, including print, electronic (web-based), social media material for educational, promotional and marketing purposes. This authorization shall extend to all languages, media, formats and markets now known or later discovered. This authorization shall continue indefinitely unless you revoke this authorization in writing. You waive your right to inspect or approve any finish product in which your likeness is used. You will make no monetary or other claim in connection with the authorized use of your name or photos, video, interviews, and quotes.

You acknowledge that you have read and fully understand the General Release Form. You release Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., and their partner organizations and employee of all liability. This agreement is a release of all liability and a contract enforceable against you (and your heirs, legal representatives, guardian, executors, successors, and assigns) in a court of law. Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., reserves the right to revise these regulations and/or terminate this agreement at its sole discretion. Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., reserves the right to remove any displays or deny entry to any group(s) that do not meet the standards set forth by Queen City Ivy & Pearls Foundation & Charlotte (NC) Alumni Chapter of Kappa Alpha Psi Fraternity Inc., and their partner organizations employee by in the guidelines. You understand that there are no refunds or rain checks due to inclement weather or any other circumstances.
