We will be at Dobie Middle School on June 12th from 10AM-2PM administering the Pfizer Vaccine. Everyone 12 years of age and older can receive the COVID-19 Vaccine!
NO APPOINTMENT OR SIGN UP IS REQUIRED!
All COVID-19 Vaccines are free. Minors must be the age of 12 on the day of the event and have a parent or guardian present.
Everyone over the age of 18, will need to bring the completed forms, your state ID or driver’s license, and your insurance card. This is required to receive your vaccine. For minors that do not have a State ID or Driver’s License, please bring your insurance card with their name on it and/or Social Security Number.
If you are receiving your second dose, please ensure that you bring you CDC COVID-19 Vaccine Card.
