Waiver, Release, and Assumption of Risk

I understand that participation in this event may include physical activities, sports, games, recreational activities, walking, running, basketball, football, wheelchair basketball, contests, vendor areas, and other event-related activities.

I understand that participation may involve risks, including but not limited to falls, injuries, contact with other participants, dehydration, illness, property loss, or other unexpected incidents.

By signing this form, I voluntarily agree to participate, or allow my child to participate, in this event. I accept full responsibility for any risks connected to participation.

I release and hold harmless the event organizers, sponsors, hosts, volunteers, coaches, staff, partners, vendors, property owners, and affiliates from any claims, injuries, damages, losses, or expenses connected to participation in this event, except in cases of gross negligence or intentional misconduct.

Medical Consent

In the event of an emergency, I authorize event staff, volunteers, coaches, or medical personnel to seek emergency medical assistance for myself or my child if needed.

I understand that every effort will be made to contact the emergency contact listed above. I accept responsibility for any medical expenses that may result from emergency treatment.

Photo and Video Release

I give permission for photos and videos taken during the event to be used for event promotion, social media, marketing, newsletters, websites, press, and future community outreach.

Code of Conduct

I understand that all participants, parents, guardians, and guests are expected to act respectfully and safely.

The event organizers reserve the right to remove anyone from the event for unsafe behavior, fighting, harassment, threats, disrespect toward staff or participants, or failure to follow event rules.

By registering for the event I agree to all the above statements.
