A presentation at the Western Region Aging Life Care Association conference by long term care planning specialist & certified care manager Jennifer Crowley.The amount of data the Aging Life Care Professional must collect and organize can seem overwhelming. The Aging Life Care Professional may not have a process for prioritizing needs and tracking progress to address the dynamic needs of the client. Care planning can be an essential component for working with clients to help address issues, concerns, and wishes and developing results that help improve outcomes.
Participants will gain familiarity with concepts and methodology in care planning, identify common challenges and helpful strategies on how to address them, and participate in guided discussion using case examples. Participants will gain knowledge in utilizing the CREATE method for effective care planning, helping improve efficiency and confidence when working with clients from start to finish.
