ECM is a statewide Medi-Cal benefit for members with complex needs, offering person-centered care coordination through a Lead Care Manager
What is the Program of All-Inclusive Care for the Elderly (PACE)?
PACE is a coordinated care model where an interdisciplinary team provides medical care, long-term services, and social supports to help older adults live independently in their communities
ECM and PACE can help skilled nursing facilities connect eligible residents with the support they need

Sharon is a geriatric healthcare leader with 20+ years of experience and a passionate advocate for the PACE model and coordinated care for older adults
