Welcome to the celebration of **CADAprograms**' 34th year and the retirement of our beloved colleague, **Dr. Janis C. Brooks**! Join us for a memorable afternoon filled with laughter, joy, and fond reminiscences. Let's toast to the accomplishments of **CADAprograms** over the past three decades and raise our glasses to honor the dedication and service of **Dr. Brooks**. This event will be in person, so get ready for an afternoon of great company and heartwarming moments. We look forward to seeing you there!

INSTRUCTIONS FOR PURCHASING TICKETS, MAKING SPONSORSHIP DONATIONS AND REGISTERING GUESTS ARE PROVIDED BELOW, SO PLEASE READ THIS ENTIRE PAGE CAREFULLY.

PURCHASING YOUR TICKETS - You may purchase general admission tickets or make a sponsorship donation at different levels. Each ticket type and sponsorship levels have promo codes that you must use to get your tickets or make your sponsorship donation. We will stop selling tickets once event capacity has been reached. Please follow the instructions below:

To Purchase General Admission Tickets:

1. Click "Get Tickets". In the "Access Code box", type "GENERALADMISSION" , click "Apply" and select the number of tickets that you desire.
2. Click the "Checkout" button. It is on this page where you register the people for whom you purchased tickets. For each guests, please indicate their meal preference for entrées and desserts to complete their registration by 9/20/2024:

Choice of one entrée:

• Stuffed Chicken Breast with Spinach, Mushrooms and Gruyere with a Balsamic Glaze

• Braised Beef Short Rib, Slow Cooked Boneless Beef Short Rib with Red Wine Sauce

• Ginger and Honey Glazed Salmon, Finished with a Lemon Mascarpone Sauce

Choice of Dessert:

• Triple Chocolate Cake or Peach & Berry Pie

Finally, choose your payment method and click "Place Order" at the bottom of the page. Credit cards and checks are accepted. A confirmation email and etickets will be sent for registered guests.

SPONSORSHIP DONATIONS AND PERKS

Take advantage of the opportunity to become a sponsor of CADAprograms! If you choose to be a sponsor, you or your organization will receive complimentary tickets for a specific number of guests of your choice and other perks based on your sponsorship level (see flier below).

SPONSORSHIP DONATIONS AND GUEST REGISTRATION

To checkout as a sponsor:

1. Locate your sonsorship level PROMO code below that is listed next to your sponsorship level.
2. Click "Get Tickets" and enter your PROMO in the "Access Code" box at the top of the page after you click and then click "Apply". Two boxes will appear.
3. Click the "+" button in the "Sponsor Donation" box and you will see a dollar amount that represents your sponsorship donation. You will also see the perks that accompany your sponsorship donation level.
4. In the next box, set the number of "Complimentary" tickets to be 1 less than the number of complimentary tickets you get with your donation. For example, if you donate $10,000 and get 16 complimentary tickets, select only 15 complimentary tickets in this box.
5. Click the "Checkout" button. It is on this page where you will register yourself and your guests and select meal options. Registration must be completed by 9/20/2024.
6. For your individual guests, please obtain their meal preference for entrées and desserts and check the corresponding boxes. Meal choices are as follows:
