URGENT(Read below)

Please bring the following to your appointment:

• Photo ID
• Social Security Card
• Proof of ALL Income (s) for each individual to be on application - pay stubs (copies of 6 most recent consecutive paystubs), social security award letter, pension letter, etc (If it applies).
• Names and DOBs for ALL individuals over 18 to be on application

• Have and be ready to access your e-mail account
