About this Event

We invite you to attend our Advance Care Planning workshop at the Cancer Center. Please note, this session is only for Cancer Center or Palliative Care program affiliated patients. Please contact Sally Bird Clough, LICSW with questions - 360-788-8232.

Join us for a free interactive workshop on completing a medical Advance Directive. All participants will be guided through this simple yet thought provoking process of choosing a Health Care Agent and putting your wishes in writing related to your health care in the event that you are unable to speak for yourself. All documents will be provided, and a Notary Public will be available to notarize completed documents for attendees if desired.
