You do not need to fill out the form if you have done so in the past.

The form will show a fifty-dollar donation request for a Thirsty! social club membership. This donation is not required to attend. However, a completed liability waiver form is mandatory for entry.
Upon arrival, a host will confirm your completion of the form and provide a wristband. Anyone found inside the venue without a wristband will be asked to complete the waiver form or be escorted out.

*PARKING: please do not park in any lots in front of or around the venue. Any unauthorized parked cars will be towed. Please park on the street only. Do not leave any valuables in you cars.
*LOITERING: please do not loiter in front of or around the venue. We do not want to bring attention to the event and get shut down due to a complaint from neighbors.
*SMOKING, no smoking of any kind within the venue. There will be a small designated outside smoking area.
