An ACP is an important document that describes what is important to you, as well as the healthcare and treatments you want as you near the end of your life. This plan will make it help the health care team caring for you, and your whanau and loved ones make decisions about your care if you can no longer tell them what you want. Unlike a will which becomes active after you die, the ACP identifies your wishes before you die.
