We are excited to offer this FREE opportunity for all middle and high school student athletes in the Acadiana region to get their annual physical completed. Each student athlete must have LHSAA Medical History form completed and signed by a parent before arriving. You can download the form here: https://www.lhsaa.org/siteuploads/editorimg/file/Sports%20Medicine/MedicalHistoryMedicalExam.pdf Scheduled times are recommended, but student athletes can arrive at their time of convenience until 5:30. Scheduled times are as follows:
• Acadiana High School, Lafayette High School and Comeaux High School 2:30 p.m.
• Southside High School, Carencro High School, Northside High School and David Thibodaux STEM Magnet Academy 3:00 p.m.
• Private, Independent and Charter Schools 3:30 p.m.
• Lafayette Parish Public Middle Schools 4:00 p.m.
• Acadia, St. Martin and Vermilion Parish Public Middle and High Schools 4:30 p.m.
