2026 Back-to-School Health Fair

Memorial Primary Care's Back-to-School Health Fair will offer free immunizations, health screenings, and school physical exams.
About this Event

Memorial Primary Care is excited to host its annual Back-to-School Health Fair on Saturday, August 1, from 9 am - 2 pm, at Memorial Primary Care Center on Pembroke Road in Hollywood. Free immunizations, health screenings, and school physical exams will be offered to uninsured and underinsured children ages 4–17.

Enjoy free health screenings and fun activities:

• School immunizations
• Broward County Schools physical exams (no sports physical exams)
• Vision and hearing screenings
• Kids activities and entertainment
• Live DJ
• Free book bags for children receiving services (while supplies last)

Important Notes

• The first 100 children are guaranteed to receive services. The remaining children will be seen as time permits up until 2 pm.
• Parents/Legal guardians must bring a valid photo ID, or their child will not be eligible for services. If possible, bring your child's ID (e.g., birth certificate, passport, or school ID).
• If you won't be with your children during the health fair, parents/legal guardians must complete the Kinship Letter to Treat form.
• A copy of the child's current immunization record is required for immunizations.
• Immunizations and physicals are provided for Medicaid patients and children without health insurance, ages 4-17.
• If you have a child with special needs, contact us for further arrangements.
• If your child is insured, please call 954-276-1200, and we would be happy to schedule an appointment for them to be seen at our Memorial Primary Care Pediatric Office.
