$10 Sport Physicals sponsored by Northern Arizona Healthcare’s Orthopedic & Spine Institute (OSI), primary care and EntireCare Rehab & Sports Medicine.All proceeds are donated back to the students' school to benefit youth athletic department programs. Cash or check payment is accepted. Checks should be made out to Northern Arizona Healthcare.
Student-athletes under the age of 18 need to have a parent or guardian present. Please fill out forms 15.7A and 15.7B found on www.aiaonline.org/about/forms before arrival.
